MEMBERSHIP REQUEST
As of the date hereof, to be admitted as an ordinary member of the Super Captains Team Association and to be entitled to exercise the rights and privileges provided by the association's bylaws. To this end, he declares as of now, to know and approve without reservation the bylaws of Super Captains Team and to join the same by assuming, along with the rights, the consequent obligations, in particular to undertake to pay membership and contractual assistance contributions in the measures and forms determined by the Statutory Bodies either directly or through institutions affiliated under Law 04.06.73 nr. 311 and subsequent amendments where applicable. In the latter case, it allows the collection of membership and contractual assistance contributions to take place at the same time and in the same manner as for the payment of the mandatory contributions established by the rules of Law. With regard to the commitment made, the undersigned is aware that membership is valid for one year, is therefore not divisible and is understood to be tacitly renewed from year to year unless revoked. He/she is also aware that any revocation, explicitly made, shall take effect on January 1 of the following year, provided it is received by September 30 of the current year. This application is subject to acceptance by resolution of Super Captains Team. He/she also applies for admission as an ordinary member of the Super Captains Team association and of any relevant associations or confederations to which it may belong such as Confsal Fisals.
CONSENT TO THE PROCESSING OF PERSONAL DATA
The undersigned, having read the information on the processing of personal data, set out at the back of this document, expresses his or her consent to the processing of personal data concerning him or her, including "sensitive" data of Reg.UE679/2016 for the purposes and in the manner indicated in the same information, including for the purpose of the communication of the same, within the limits indicated.
He also declares, having read the information on the processing of personal data, Reg.UE679/2016, to consent to the processing of personal data concerning him for purposes related to direct marketing activities (sending information and promotional material) by Companies affiliated with the Association Owner of the Treatment, Super Captains Team always within the scope of the activities carried out by it and for the purposes stated in the same information.

Nature of data processed
In addition to your personal and tax identification data, defined by law as "common data" for the proper conduct of the employment and contractual relationship, we need to process certain data defined by law as "sensitive": in particular, these are those data from which we obtain information on membership in trade organizations and unions, to the extent that the processing of such information is necessary for the pursuit of purposes determined and identified by the deed of incorporation and the federal statute Super Captains Team. We specify that in processing such data we will scrupulously adhere to the limits and conditions imp osed by the Privacy Guarantor in authorization No. 3/2012 for the processing of sensitive data by association-type bodies or foundations.
Purpose of processing
Common/sensitive personal data will be processed for the pursuit of the determined elegitimate purposes identified in the art icles of incorporation and federal bylaws:
• Survey of the degree of representativeness of the association;
• Collection of membership and contractual assistance contributions;
• Creation of the membership file forsending communications, information, business proposals and convening of statutory bodies;
• Activities of assistance and representation to members as provided for in the association's bylaws;
• Professional and nonprofessional, technical and union training activities aimed at or for the benefit of members;
• Study activities and implementation of initiatives directed toward union, social, economic, and political assistance to members;
• Labor contract consulting activities: assisting with labor contracts and resolving any disputes;
• Activities to represent members in entering into collective bargaining agreements;
• Designation and appointment of its representatives to bodies, boards and commissions;
• Work safety, legal, tax, administrative, accounting and financial assistance activities aimed at members;
• Performance of any other duties that are entrusted to the Association by law or by resolution of the statutory bodies.
Mode of treatment
Data will be processed through the use of appropriate tools and procedures to ensure their security and confidentiality and may be carried out through the use of both paper and electronic media.
Access to and changes in data are permitted only to personnel formally appointed by the data controller.
Obligation or option to give data and consequences of refusal
Although it is optional for you to provide us with your personal data, we inform you that in their absence it will not be possible for the Association to establish and continue a membership relationship. For the purpose of correct and up-to-date data processing, timely communication of any changes in the data provided is necessary on the part of the interested party.
Communication and dissemination of data
Your personal data will not be "disseminated," i.e., made known to unidentified parties, but will be communicated to parties defined as specified below:
• related bodies in charge of individual technical assistance and advisory functions;
• service companies and other third parties closely related to the Association;
• social security and welfare institutions as well as to other bodies responsible for issuing opinions and authorizations (for example: bilateral bodies, dtl, province, harbor master's office);
• Public and private entities that can access the data by virtue of a provision of the law or regulation within the limits provided by these rules: social security, welfare, insurance institutions and entities, welfare/assistance funds, public administration);
• to individuals who are our consultants to the extent necessary to carry out their duties on behalf of the Association subject to our mandate which includes the obligation of data security and confidentiality;
• to employees or collaborators of the Association upon appointment as "data processor."
HIS/HER RIGHTS:
We reproduce below the excerpt from EU Reg. 679/2016 to remind you that at any time you may exercise your rights against us, in particular:
• to know the period of retention of personal data or, if this is not possible, the criteria used to determine this period;
• obtain the cancellation of the same or the restriction of the processing concerning them or to object to their processing, as well as the right to data portability;
• revoke consent at any time without affecting the lawfulness of processing based on the consent given before revocation;
• Propose a complaint to a supervisory authority.
